Case Manager - Civil Citation and Outpatient
Responsibilties
Assess and manage all assigned cases and complete daily documentation of contact with clients, families, and others through agency case note. Conduct case management services for clients and families in Civil Citation and Outpatient programs, which include but are not limited to, making referrals, scheduling appointments, and monitoring treatment/service recommendations.Demonstrate skills in assessment of clients referred for diversion services using Agency Screening and Assessment tools such as the Global Appraisal of Individual Needs (GAIN) tool.
Requirements
Minimum of a Bachelors degree from an accredited college or university with course work in social or behavioral science field. At least one year of experience in the juvenile justice, mental health, and/or chemical dependency field is preferred. Master’s degree may be substituted for one year of experience.
Salary Rate
$20.00 per hour
