We are looking for a Case Manager to join our Tallahassee/Leon County Office. In this role, you will work directly with people served to ensure they receive the care they need to support their recovery efforts.

Essential Responsibilities:

Conduct case management services for individuals and families. These services include, but are not limited to, making referrals, scheduling appointments, and monitoring treatment/service recommendations. Conduct thorough assessments using agency-approved tools (e.g., Biopsychosocial Assessment) and implement appropriate service plans aligned with the Wraparound model for care coordination.  Identify, engage, and enroll eligible individuals through targeted outreach; connect clients to appropriate treatment, recovery, and support services.  Complete daily documentation of contact with individuals, families, and others through agency case notes. Facilitate or support educational sessions on substance use, life skills, employability, and vocational development. Collaborate with community organizations and external partners to enhance service delivery and resource access. Travel locally as needed to meet with clients, attend trainings, and participate in conferences.

Requirements:

Bachelor’s degree is required in a Social or Behavioral Science field. Strong verbal and written communication skills. Performs work independently with minimal supervision. Capable of multi-tasking, highly organized, with excellent time management skills. Strict adherence to confidentiality with high ethical standards. A valid Driver’s License and proof of insurance are required.

Salary Rate

$20.00 per hour
