DISC Child Welfare Case Management programs provide prevention and intervention services to reach safety, security, and well-being for dependent children. These services are provided to children and families whether they reside in the home with their parents or have been moved into out-of-home care and residing with relative/non-relative placements. Certified Dependency Case Managers work collectively with families to establish goals, creating safety and case plans to achieve said goals, providing services to meet needs identified in ongoing assessments, monitoring progress toward achievement of the goals, and closing cases when goals have been achieved. While establishing goals, the early intervention and assessment addresses the reason for involvement with the Child Welfare System of Care with the primary goal of remedying those concerns and increasing caregiver protective capacities.
