  • Re Entry Case Manager - Madison

    RESPONSIBILITIES:
    Administer a Comprehensive Addictions and Psychological Evaluation – 5 (CAPPE-5) for each individual admitted to the detention facility. Facilitate substance education and re-entry classes, and provide pre/post-release case management in a correctional setting. Sessions will be appropriately and accurately documented on logs and progress notes. Coordinate with other community-based providers, including both public and private, to provide additional services for the client. Services may be provided on evenings and weekends as needed.
    REQUIREMENTS:
    Individual must have a minimum of a bachelors degree in a social or behavioral science field and one (1) year of experience in case management and/or counseling is preferred. Seeking a motivated self-starter to assist adults transitioning from a correctional setting. Excellent verbal and written communications skills. Strong organizational, time management and problem solving skills. Ability to be flexible and dependable, and demonstrate initiative and creativity. Experience working with diverse populations, particularly reentry and adults with economic disadvantages, preferred
    Salary Rate: $16.35 per hour
