Re Entry Case Manager - Madison
Responsibilties
Administer a Comprehensive Addictions and Psychological Evaluation – 5 (CAPPE-5) for each individual admitted to the detention facility. Facilitate substance education and re-entry classes, and provide pre/post-release case management in a correctional setting. Sessions will be appropriately and accurately documented on logs and progress notes. Coordinate with other community-based providers, including both public and private, to provide additional services for the client. Services may be provided on evenings and weekends as needed.
Requirements
Individual must have a minimum of a bachelors degree in a social or behavioral science field and one (1) year of experience in case management and/or counseling is preferred. Seeking a motivated self-starter to assist adults transitioning from a correctional setting. Excellent verbal and written communications skills. Strong organizational, time management and problem solving skills. Ability to be flexible and dependable, and demonstrate initiative and creativity. Salary Rate
$16.35 per hour
