Case Manager - Franklin
Responsibilties
As a Case Manager, you will work directly with clients to ensure they are connected to the care they need to support their recovery efforts. You will be responsible for assessing clients, scheduling appointments, making referrals, and monitoring treatment. This position requires a high level of communication and organizational skills as well as the ability to connect with clients from different backgrounds.
Requirements
A minimum of a bachelor’s degree from an accredited college or university with major course work preferably in a Social or Behavioral Science field with at least one year of case management experience in the juvenile justice, mental health, and/or substance use disorder field preferred.
Salary Rate
$20.00 per hour
